We attempt to accomplish this by daily featuring Literature artists from around the community that deserve the recognition, but are not getting it. I use the circle selection, then add a tail. Colour white. New Layer, Go back to the circle selection tool, then right click and go stroke. Flip layers around depending on how you want the line to look, but all done.
Or, even more lazy, use the speech bubble shape, then go transform and distort it to the text.
I dunno how to create the perfect curve on the tail yet though.
